where are the initial reports bullet fragments were recovered and reported to law enforcement? Shouldn't there be some paper trail/record from the medics that handled meg's foot the night of the incident? did they lose those records as well?

That was also over 25 years ago. Technology and processes have come a long long way since then. That kinda stuff just doesn't happen anymore in cases like this

It happens everyday lol yes, we've made changes from that trial, but at the end of the day a human still has to collect the evidence and put it into their system. It could easily be errors in that.
