Best to avoid both (And anything that reduces fever) if possible. Let the fever run it’s course unless you cannot tolerate it. If it’s high enough where you absolutely need to reduce it, you should probably be in the ER. Fevers are one of the body’s natural defense mechanisms against infection. Fever will treat the disease. Pain killers will not. Reducing fever will lengthen and possibly worsen the course of the illness.

Of course, if you absolutely have to take a pain killer/fever reducer, acetaminophen is probably your best bet. While saying it’s not an NSAID isn’t entirely true, it likely works only on inflammatory enzymes in the CNS (I say likely because we aren’t 100% sure of its mechanism of action). That means it likely won’t directly interfere with any inflammatory responses your body is targeting at COVID.
