Yeah it happens pretty often, but it usually gets sorted out
But once enough untampered files from a specific era leak, you can go back and crosscheck that the file naming scheme and metadata formatting is consistent between new leaks and old leaks. Like if it's known info that versions 1, 2 and 3 of song "x" came from a folder called "y" on person "z's" laptop, and they are all named/tagged the same way, and then versions 4 & 5 of that same song leak and they're named/tagged the same way, you'll know it came from the same folder, and can date it to the same time
Leakers also check each other. So if Seller "A" is selling version 18 of song "B", and claiming it's from Yeezus 2, other sellers that have sourced Yeezus 2 can confirm or deny what seller A is saying

It goes beyond this too
They can tell if people are trying to spoof the metadata/tagging to make it look like it came from a specific era
They check things like the bit rate / bit depth, software the track was bounced from, the specific version of that software, etc.
So if tracks 1, 2 and 3 come from folder A, and a seller is claiming that song 4 also comes from folder A, once that song leaks, even if the metadata/tagging looks generally correct, if 1, 2 and 3 were bounced from Protools v1.4.5 and song 4 was bounced from Protools v1.8.7, you know they're faking it
