In 2003, NBA agent Bill Duffy somehow forgot to notify the Miami Heat that his client, Anthony Carter, was going to exercise his $4.1m player option. This made Carter a free agent, and since he wasn't a very good player all he could get was a minimum contract with the Spurs ($750,000). To make up for this, Duffy worked out a 17-year payment plan to pay back $3m to Carter. This year he made the final payment.

This mistake didn't cost Duffy his career, and he's currently the agent for Luka Doncic, Rajon Rondon and Goran Dragic.
