Immersion. Being exposed to the language will speed up the process more than anything. Put your computer in spanish, phone etc. Read the news in spanish, watch programs in spanish (I don't recommend watching American shows with spanish dubs tho), listen to spanish podcast. Basically have to drown yourself in spanish. Will be uncomfortable at first, but this is how you force yourself to think in the language.

Also gotta be constantly acquiring new grammar. Using flash cards and stuff is fine (theres a nice free program called Anki, you can add your own stuff or download other decks people have made, also supports plenty of addons), but the best way to learn knew stuff is to learn it IN context that's used.

But before any of that, you need a good Spanish Grammar book (I reccomend English Grammar for Students of Spanish by Emily Spinelli. It's cheap but does a great job of explaining).
